Commercial Application - 3 Part Form

Use this form for all Commercial (Ohio Building Code regulated) permits except electrical.  You must complete Parts A & B for all jobs, and Part C must be completed by the designer of record for all Building, Fire Suppression, and Alarm System Permits except those for full building demolition.  Part C is not required for Mechanical permit applications.
